Is the medical form that is found on the CKRC website a requirement to be completed before a race?

Any suggestions on where to purchase boots for kart racing in Canada? We need a pair for a 13 year old and don't want to pay outrageous prices since he will grow out of them by next year. Any help would be appreciated.
